Telehealth uses computer and video technology to connect your child at school to a doctor or nurse practitioner at Children's Health Pediatric Group.

The front office or the nurse's office has enrollment forms available. Enrollment forms can be returned to the front office or school nurse.

Yes. A new form must be submitted at the beginning of each school year.

An After-Visit Summary can be provided to you and your child's primary care pediatrician to continue treatment as necessary.

If you have commercial insurance or Medicaid, Children's Health5M will collect and verify coverage information. If you don't have insurance or if your insurance provider does not cover Telehealth visits, we encourage you to continue to use Telehealth, and we will send you an invoice of no more than $50 after your child's visit. For all payers there are discounts available when you pay at time of visit. If you have questions concerning billing, please call (844)-4TELEMED, Option 1.

If your child is enrolled and gets sick at school:

• The school nurse will determine if a pediatrician is needed to diagnose the child, based on Telehealth guidelines.
• The parent will be contacted with appointment information and invited to participate by phone by Children's Health.
• An appointment will be set up with the pediatrician.
• The school nurse and pediatrician will conduct the exam.
• The pediatrician will call in a prescription to your chosen pharmacy if necessary.
• An After-Visit Summary will be provided to the parent.
• If your child is not a Children's Health Pediatric Group patient, we recommend the parent/guardian shares the After-Visit Summary with their child's pediatrician.

Children's Health uses MyChart®, a web-based medical records system. This provides a way for parents to view information about a Telehealth visit online. Please visit childrens.com/mychart for more information.

If we have a consent to bill on file, the school nurse may move forward with the appointment. If there is no consent on file, the school nurse will not conduct the appointment until the Children's Health team has connected with the parent/guardian.

In 2014, Children's Health launched this program and was able to fund it under a limited amount of state dollars. Our primary goal has always been to demonstrate to lawmakers and parents that we could improve health outcomes in children by using innovative technology to care for them in their schools. Because of your partnership, we have accomplished that goal. In just three short years, we have:

• Treated more than 6,000 patients
• Expanded into 97 schools, 16 school districts, enrolling approximately 22,000 students
• Become the fastest-growing school-based telemedicine program in the nation
• Achieved 91% satisfaction rate from parents

We will continue to treat all enrolled children - regardless of coverage. If a patient has commercial insurance or Medicaid, Children's Health will collect and verify coverage information. If they not have commercial insurance or Medicaid, or if their insurance provider does not cover Telehealth visits, we encourage them to continue to use Telehealth, and we will send them an invoice of no more than $50 after the visit. By billing commercial insurance payors and Medicaid for services rendered, we will be able to sustain and further grow this program so we can provide care for all children.
